Before Plate Activation

Truck Title — Front & Back

Mandatory

You must provide clear photos of BOTH sides of the truck title.

Title Requirements:

  • Front and back must be visible
  • VIN must match the truck
  • Owner name must be clear
  • No blurry, cropped, or edited images
  • Title must be clean (no unresolved liens unless approved)

Only clean, readable titles will be accepted. Sample images show acceptable quality standards.

Form 2290 — Highway Use Tax

Mandatory

A valid IRS Form 2290 is required for the unit being plated.

2290 Requirements:

  • Must be current for the tax year
  • VIN must match the truck
  • IRS-stamped or IRS-accepted copy only
  • PDF or clear photo accepted

All plates and devices remain company property.

Depending on your setup:

The company may issue different combinations of equipment based on your specific operational needs and compliance requirements. Everything is coordinated during onboarding.

Termination & Return Policy

Clear rules for professional separation

If the driver leaves or is terminated:

Plate becomes inactive immediately

As soon as termination occurs, the plate is deactivated

Must return plate and all devices

Driver must return the plate and all company-issued equipment

Keeping or using plate = DOT violation

Using the plate after termination is illegal and punishable

Failure to Return Items May Result In:

  • Settlement deductions for unreturned items
  • Replacement charges for missing equipment
  • Compliance action if necessary
